The thing about seatbelts is they weren't designed to fit all people...especially not shorter people.

Some car manufacturers have made the seatbelt connection on the door pillar adjustable in an effort to help solve this hazard of seatbelts against the neck.  But even these adjustable belt attachments do not help many shorter people. 

During an accident, the seatbelt will retract and lock against the neck. The upper body pitches forward in the early stage of an accident, and is supposed to be restrained by the upper portion of the seatbelt. Car manufacturers designed the belts to cross the upper body between the shoulder and the neck. Even the collarbone and shoulder can be damaged from the extreme forces encountered during an accident. But the neck is more delicate. If the seatbelt is positioned in a way that transfers its restraining force to the neck, more serious damage can result. 

The other problem with having a seatbelt rubbing against the neck is one familiar to a great many women and shorter people. It's uncomfortable and irritating, especially during a long trip where the edge of the seatbelt continually rubs against the neck. 

Everyone knows wearing a seatbelt can save your life in an accident. In fact, today almost all states have laws requiring drivers and passengers to wear seatbelts. But the discomfort and irritation of having the belt rub against the neck causes many people to wear the belt improperly, and sometimes not wear it at all. Some of  those suffering from the seatbelt chafing against the neck put the diagonal portion of the belt, which is supposed to restrain the upper body, behind the under their arm or even behind their back.

But if the seatbelt were comfortable, and could be properly positioned away from the neck, these people would not have to defeat the seatbelt's purpose to be comfortable. The slack lets the body pitch forward into the detonating airbag. The danger of being close to a detonating airbag has been well documented and is the reason NHTSA gives permission for shorter drivers, who are closer than 10 inches from the steering wheel to have an airbag on-off switch so they can shut the airbag off for safety. 

Seatbelts were designed to restrain the body at some of its strongest points: at the hips and the middle of the shoulder. If these locations change, there is a danger of injury to internal organs. NHTSA has published a notice that warns of the danger from a fabric adjuster, which looks like a sling and covers the wearer's seatbelt. The seatbelt sling cover changes these restraint points so the force of the body during an accident is taken up by the chest and stomach.
